Job Coach Description
JOB SUMMARY
Job Coach role is to assist & support individuals with ABI/TBI or other disabilities to perform assigned job task and learn related work skills to become independent and acclimated through vocational training and community interaction. A. Job Coach provides ongoing support to consumers, it is individual-based and centered around work-related goals that focuses on helping the consumer obtain the maximum level of independence & confidence to perform their job and pursue their professional goals.
As a Vocational Job Coach in this program you, will be teaching, queuing, directing, coaching and supervising clients with acquired brain injury or other disabilities as they work on achieving independent life skills through work skill programs, community involvement and Prevocational activities at the NDHC Supported Employment Job Center.
The hours of operation are Monday – Friday from 9:00 A.M. to 3:00 P.M.
QUALIFICATIONS & SKILLS
- Excellent Communication, Interpersonal & Customer service skills
- Ability to engage consumers from diverse social and ethnic background
- Must be CPR certified or able to obtain within 3 months of employment
- Clean DMV record, reliable transportation & valid registration and insurance coverage
- Proficient computer skills including Microsoft Office, Word, Excel & Outlook
- Certified ILST or Recovery Assistant is a plus; or previous experience as a Job Coach
- College Degree in Social Services or High School Diploma or equivalent training
- Ability to inspire and motivate consumer with positive attitude & patience
- Ability to observe and compile weekly progress report on consumers
- Ability to cope and remain calm in emergency situations (i.e. accidents, CPR, seizures)
- Ability to work independently and as a team player and to work cooperatively with co-worker and Supervisor.
RESPONSIBILITIES & DUTIES
- Coach a group of 6 adults with developmental disabilities or acquired brain injury in our Prevocational and Employment Assistant Programs.
- Communicate with clients to understand their goals and ambitions
- Conduct scheduled Prevocational Activity with clients and document daily progress
- Assist clients to discover and overcome their personal barriers and set goals
- Transport clients to designated locations on time using business or personal vehicle for program approved activities or appointments
- Assist clients in developing social, work and life skills and document their progress
- Assess strengths of clients and teach them how to use them effectively
- Maintain confidentiality & safeguard client’s Personal Information with co-workers or guest
- Compliance with HIPAA Policy as it relates to client’s PHI records while on site or travelling
- Direct, coach & support clients as they perform assigned duties at Job Center or other offsite
Employment locations
- Create/update/manage schedules for your assigned group or individual client
- Prepare daily progress notes on client achievement/behavior to be used for quarterly reporting.
- Provide support & motivation to clients as they work to regain independence and redirect inappropriate behavior
- Communicate any concerns promptly to the Case Manager and make necessary referral
- Assist client in the job Application Process at the job center or external approved employer
- Create & produce weekly Client Activity Schedule and submit to Supervisor for approval
- Execute scheduled activities in set time frame as indicated on Supported Employment Work Calendar posted at the Job Center
- Any other related duties that maybe assigned by Supervisor/Manager or Senior Management
