ILST Job Description
JOB SUMMARY:
An Independent Living Skills Trainer (ILST) position is a One on One-person centered service designed to improve the ability of the consumer to live independently in the community.
The ILST provides non-medical care, supervision, and socialization in accordance with the client’s Service/Care Plan. Services are provided in the consumer’s home or in a community setting
QUALIFICATIONS
- Individuals who have completed two (2) years of Home & Community support services or residential habilitation under the office of Mental Retardation & developmental Disabilities waiver
- Must maintain a valid CT’s driver’s license, reliable transportation, good driving record, and proof of insurance.
- Bachelor’s degree in Human Services and 1 year of experience working with consumer with disability or High school diploma with PCA certification and two years of related experience
- Strong organization, communication, and interpersonal skills
- Ability to observe, evaluate, document, and communicate verbally and in writing, client performance and social behavior.
- Maturity and ability to deal effectively with the demands of the job.
- Ensure service is delivered in accordance with company’s policies & procedure
- Must be able to follow direction and accurately report to the Supervisor /Office Administrator with any new information concerning the client.
RESPONSIBILITIES:
The responsibilities of the ILST may include, but are not limited to the following:
- Facilitate the learning of independence through promoting skills such as cooking, cleaning, financial management and community integration.
- Assist with issues related to self-care, medication management, task completion, communication, and social skills, pre-vocational and other activities of daily living.
- Ensure continuity of consumer services and Agency standards of client care.
- Facilitate consumer service/Care Plan and implementation of goals in collaboration with the treatment team.
- Support and encourage consumer to develop independent living by acquiring skills needed to maintain their home and access their community, such as problem solving, organizational personal & interpersonal skills
- Provide transportation with personal vehicle as required by Program and ensure client safety and welfare during loading, transit and unloading.
- Provide direct line of sight supervision to the consumer to ensure the continued delivery of quality client focused services while promoting their independence, health, and safety
- Advocate on behalf of consumer by providing intervention and consultation as needed.
- Assess and document consumer progress daily in Agency Client Service Delivery Logbook and perform quality control.
- Support & encourage consumer daily to maintain personal hygiene care.
